SUNUM ÖZETİ

Yenidoğanlarda klitoromegali daima patolojiktir ve intrauterin dönemde androjen maruziyetini gösterir. Klitorisin normal değerlerini bilmek, tanı koymak için en temel bilgidir. Literatürde yenidoğan döneminde klitoris boyutları ile ilgili az sayıda çalışma vardır.

Bu çalışma Türkiye’de yapılan, yenidoğan klitoris boyu ölçümünü içeren ilk çalışmadır.

2008 yılı ilk altı ayı içinde, NSVY ile doğan 340 kız bebek çalışmaya alındı. Gebelikte ilaç kullanımı ya da annede hirsutizm olanlar, prematürite ve SGA bebekler çalışma dışı bırakıldı.

Vucut ağırlığı, boy ve baş çevresi ölçüldü ve klitoris uzunluğu kayıt altına alındı.Tüm ölçümler tek bir araştırmacı tarafından yapıldı. Verileri eksiksiz olan 325 bebek istatistiki değerlendirmeye alındı.

İstatistik değerlendirme SPSS 11.5 ile yapıldı. Pearson korelasyon, ROC analizi, Multipl Linear Regresyon, Coefficient of Variation ve Inter-Class Correlation Coefficient  uygulamaları yapıldı.

Klitoris uzunluğunu hesaplayabilmek için formül üretildi. Klitoris persantil eğrisi oluşturuldu

325 hastalık seride ortalama gebelik süresi  277.6 ± 7.3 gün, boy 49.23 ± 1.82 cm, vücut ağırlığı 3.16 ± 0.37 kg, baş çevresi 34.13 ± 2.11 cm ve klitoris uzunluğu 4.93 ± 1.61 mm, olarak bulundu.

Klitoris uzunluğu ve gebelik süresi arasında ve baş çevresi arasında korelasyon yoktu. Ancak klitoris uzunluğu ile boy ve ağırlık arasında negatif korelasyon tespit edildi.

Bu veriler yenidoğan değerlendirmesi, genito-endokrin değerlendirmede kullanılabilir.

Öte yandan oluşturulan formül ile klitoris boyu hesaplanabilir.

Clitoromegaly in the newborn period is always accepted as pathological and indicates exposure of the female fetus to androgen in the intrauterine period.

There are few studies on the clitoris length in the newborn period in the literature. (1,3,5,6)

This study is the first to investigate clitoris length in newborn females in our country.

A total of 325 newborns born by uncomplicated spontaneous vaginal delivery were included in the study. The mothers had no history of hirsutism or drug use during pregnancy. The clitoris measurement was performed on the 2nd postnatal day by the same investigator. Premature or small for gestational age (SGA) patients and those with genital anomies were excluded from the study.

The weight, height and head circumference and clitoris lenght of the newborns were measured by the same investigator.

The data were analyzed with the SPSS 11.5 package software. The Pearson Correlation test Multiple Linear Regression analysis, ROC analysis, Coefficient of Variation and Inter-Class Correlation Coefficient were used.

The mean values for the 325 patients included in the study were as follows: pregnancy duration 277.6 ± 7.3 days, clitoris length, 4.93 ± 1.61 mm, height 49.23 ± 1.82 cm, weight 3.16 ± 0.37 kg and head circumference 34.13 ± 2.11 cm.

We found no correlation between clitoris length and gestational age.

No correlation was found between clitoris length and head circumference.

A negative correlation was found between clitoris length and height and weight.

A clitoral length pencentile was prepared and an equation was produce to calculate length of clitoris.

In conclusion, there may be racial differences between clitoris length values and we suggest that each country define its own normative data. The normative data produced for the newborn period can also be used for infancy and represent an important diagnostic tool for conditions that produce late clinical signs.
